1. This is a fillable PDF. Feel free to fill out on computer and then email it to info@PrestigePickleballclub.com with the subject "Kids Free Pickleball Lessons"
2. If you are not comfortable filling out on the computer, print out and bring it over to us at 160 Legrand Ave in Northvale
3. If you have more than one kid, please fill out individual forms.
4. Please get this back to us as soon as possible. Spots are limited.
